How do I know if my child needs an evaluation?

How do I schedule an evaluation?
Contact your child’s pediatrician’s office to fax a script at 904-337-4697.
Complete and sign all required documentation sent to you from a secure link.
A $50.00non-refundable deposit is required prior to scheduling. Once all documents and the deposit have been received, the speech-language pathologist will call to schedule your appointment.
Why do you collect a deposit for an evaluation?
A deposit is our effort to make certain that clients are considerate of the time we place in preparing for the appointment. It ensures that families who are truly committed to the evaluation process will obtain an appointment without lengthy waitlists. Your deposit will be applied to the total evaluation cost once the evaluation has been completed.
How long is the evaluation process?
Depending on the needs of the client, an evaluation can take anywhere between 1 - 2 hours. Our assessments include a case history, informal observations, and formal standardized testing. For Orofacial Myofunctional Evaluations, we also take photo documentation, measurements, and observe movements of the oral structures for speech and feeding. For clients 18 years of age and younger, we require that a parent/caregiver is present for the entire evaluation.

Family Empowerment Scholarship (formerly Gardiner Scholarship). We provide monthly invoices for the parent/caregiver to submit for reimbursement.

You have the right to receive a Good Faith Estimate explaining how much your medical care will cost before you receive it. Under this law, if you are uninsured or you decide not to use your health insurance for a service, healthcare providers must provide a written estimate for medical items and services. How long are therapy sessions?
The length of sessions and frequency will depend on the needs of your child. All treatment plans are individualized. Our treatment approach is family-centered. Families are valued members and are expected to be active participants in the therapy session to facilitate the learning and implementation of skills across a variety of settings.
How long will my child need therapy?
This is the most common question we receive. The length of time that your child will require therapy depends on a variety of factors such as: the type and severity of your child’s speech, language, or feeding challenges, if there are additional learning or behavioral challenges, the consistency in which your child attends therapy sessions, home practice/carryover, and motivation from the child and family.
What is your cancellation policy?
Consistent attendance and participation in treatment is essential for therapeutic success. However, we understand that children become sick and emergencies do happen. If your child is ill, or in the event of an emergency, we ask that you please call or text your clinician at least 4 hours prior to your scheduled appointment. For cancellations other than illness or emergencies, we kindly ask for 24 hours notice.
Where do you provide services?
We currently service a 10 mile radius from Fleming Island, Florida. A convenience fee will apply to locations further than 10 miles. We also service all of Florida via tele-therapy!
